Implementation of an Easy-to-use Database for the Assessment of Physician Performance in Epilepsy Care (P5.043)

OBJECTIVE: To start a prospective registry in a database fashion of patients with epilepsy to assess the adherence to guidelines of treatment, and evaluate the seizure freedom achieved by patients. BACKGROUND: There is an Epilepsy Physician Performance Measurement Set available online but it is not commonly use; it has been suggested that the use of a database structure may facilitate clinical research in epilepsy as well as patient care. DESIGN/METHODS: Our registry began on May 2th 2013 with consecutive patients older than 18 years seen in our epilepsy clinic. Demographical and clinical data included age, seizure type (ST), etiology, antiepileptic drugs (AEDs), electroencephalogram (EEG) and imaging (IRM/TC), adherence to treatment (AT), and seizure freedom (SF) Statistical analysis was perform using Sigma Stat V.3.5 RESULTS: So far, we have included 263 patients, 63 were excluded; among the 200 analyzed, the mean patient age was 33±13 years, and 115 were women, weight was 63±13kg. ST: 131 generalized, 58 focal, 11 epileptic syndromes. EEG was performed in 152 (76%) and IRM/TC in 127 (63.5%). Etiology: genetic 13 (6.5%), structural/metabolic 54 (27%), unknown 133 (66.5%). AEDs: monotherapy 104 (52%), 2-3 AEDs 93 (46.5%) y 蠅4 AEDs 3 (1.5%). AT: 161 (80.5%). SF 3 months 91 (45.5%). Monotherapy standard prescribed in 68.5%, 2 AEDs standard prescribed in 68.5%, 3-4 AEDs standard prescribed 76.6%. CONCLUSIONS: We found a great amount of patients lacking of EEG and/or MRI, which partially explains the number of patients with unknown etiology. The adherence to guidelines is moderated. Seizure freedom is achieved in half of the patients. We propose that systematization through database implementation will help to improve the adherence to treatment protocols and the performance in epilepsy care.
