Gambling disorder, previously defined as an impulse control disorder has recently been reclassified as a substance-related and addictive disorder in the DSM-5. This reclassification calls for a change in the clinical approach. This study reports the latest data on epidemiology, diagnosis, psychiatric comorbidity and pharmacological strategies. In addition, methodology and clinical approaches over a 3 year of activity of the gambling treatment team in the city of Milan, are presented. The challenges of this "new" addiction implies an update of clinical directions in dealing with and treating the disorder. The treatment system should also deal with the need to improve efficiency and accuracy in managing work processes.
