Obesity is a global challenge. At present Germany and the USA are parting leading position worldwide for obesity. Economical and individual consequences are immense. On the one hand time to treat for one single patient is high. On the other hand long-term effects are unsatisfactory and no critical factors of success have been agreed on. Interventions should most likely begin in childhood or adolescence and be performed integrated, interdisciplinary, and multimodal. Exercise respectively sportive activities appear to be one key-element to success. Prerequisites to improve quality control and quality perfecting and to obtain a good negotiating basis with health insurance companies comprise an independent external quality assurance and transparent statement of costs. The Profit Centre Concept can be an instrument to realize such transparency on a division level. Complemented by the “Adipositas Patienten Verlaufsdokumentation” (APV), it may well aid as firm negotiating basis with health insurance companies.
