ANALISIS IMPLEMENTASI KEBIJAKAN PENGENDALIAN TUBERKULOSIS RESISTEN OBAT DITINJAU DARI ASPEK KOMITMEN POLITIS PADA DINAS KESEHATAN KABUPATEN MINAHASA UTARA

Program pengendalian penyakit tuberkulosis khususnya TB Resisten Obat (TB RO) bukan hanya menjadi program nasional tetapi telah menjadi program internasional. Upaya dalam pengendalian dilakukan dengan menggunakan strategi DOTS yang memberi penekanan pada penemuan kasus dan kesembuhan pasien. Komitmen politis dari pemerintah pusat maupun daerah sangat penting dalam mencapai keberhasilan pengendalian TB RO. Saat ini struktur pembiayaan yang tersedia lebih banyak terpusat kepada aspek kuratif sedangkan pembiayaan untuk aspek promotif, preventif dan rehabilitatif masih sangat kecil. Tujuan penelitian ini yaitu untuk menganalisis bagaimana pelaksanaan kebijakan pengendalian tuberkulosis resisten obat ditinjau dari aspek komitmen politis pada dinas kesehatan Kabupaten Minahasa Utara. Penelitian ini menggunakan metode kualitatif. Teknik pengumpulan data menggunakan data primer dan data sekunder. Informan dalam penelitian ini berjumlah 7 orang. Hasil penelitian menunjukkan bahwa komitmen politis dengan peningkatan dan kesinambungan pendanaan belum maksimal, dimana pengembangan infrastruktur belum berjalan dengan baik; pengembangan sumber daya manusia belum berjalan baik: advokasi, komunikasi dan mobilisasi massa belum berjalan baik; sedangkan dukungan kebijakan pengobatan TB yang rasional sudah berjalan sesuai standar dan pengelolaan logistik belum berjalan dengan baik.  Kesimpulan dari penelitian ini menunjukkan bahwa implementasi kebijakan pengendalian tuberkulosis resisten obat ditinjau dari aspek komitmen politis pada dinas kesehatan Kabupaten Minahasa Utara belum berjalan sesuai dengan Peraturan Menteri Kesehatan Nomor 13 Tahun 2013 tentang Manajemen Terpadu Pengendalian TB RO. Komitmen politis masih sangat rendah, alokasi pembiayaan yang sangat minim, kurangnya kerjasama dengan pihak swasta, lintas sektor dan lintas program sehingga dapat menjadi salah satu penyebab rendahnya angka penemuan kasus (CDR) TB RO di Kabupaten Minahasa Utara. Kata Kunci: Implementasi Kebijakan Pengendalian Tuberkulosis, Komitmen Politis ABSTARCT The program of controlling Tuberculosis disease especially Drug-Resistant Tuberculosis is not only being a national program, but also an international program. Efforts in controlling are done by using DOTS strategy that gives emphasis on the case discovery and the healing of patients. The political commitment from the central government either the area government is very important to achieve the success of controlling Drug-Resistant Tuberculosis. Nowdays, the financing structure that is available more centrally on the curative aspect, while the financing for promotive, preventive and rehabilitative aspect is still very small. The purpose of this research is to analyze how the implementation of the policy of control of drug resistant tuberculosis from the aspect of political commitment at district health office of North Minahasa regency. This research design use qualitative methods, with the techiques of collecting data use primary and secondary data. There are seven informans in this study. The results show that political commitment with increased and sustained funding has not been maximized, where infrastructure development has not gone well; human resource development has not gone well: advocacy, communication and mass mobilization have not gone well; while the support of a rational TB treatment policy has been running according to standards and logistics management has not gone well. The conclusions of this research show that the implementation of the policy of control of drug resistant tuberculosis from the aspect of political commitment at district health office in North Minahasa regency has not been run in accordance with the Regulation of Minister of Health Number 13 of 2013 about the Programmatic Management of Drug-Resistant TB (PMDT). Political commitment is still very low, the allocation of funding is minimal, the lack of cooperation with the private parties, cross sector and cross program, so it could be one of the causes of the low figures for the case discovery of Drug-Resistant Tuberculosis CDR in North Minahasa regency.
