Cirugía valvular mitral con el corazón latiendo

En los ultimos anos ha habido un interes creciente por las operaciones de cirugia cardiaca (principalmente revascularizaciones miocardicas) con el corazon latiendo, sin pinzamiento aortico y, siempre que sea posible, sin el uso de la maquina de circulacion extracorporea, porque se ha demostrado que se pueden obtener mejores resultados que con las tecnicas tradicionales de proteccion miocardica con soluciones cardioplejicas. Este metodo nos condujo a considerar tambien la posibilidad de realizar cirugia de la valvula mitral con perfusion coronaria continua y el corazon latiendo. En el presente trabajo nos propusimos exponer la forma en la que se realizo esta tecnica quirurgica de proteccion miocardica en el periodo comprendido entre enero de 2000 y diciembre de 2001 en esta institucion y evaluar su seguridad y eficacia. Se hallo que la cirugia de la valvula mitral realizada con el corazon latiendo constituye un metodo alternativo de proteccion del miocardio, seguro y facilmente reproducible con el que se pueden alcanzar resultados clinicos satisfactorios. In the last years there has been a growing interest to carry out cardiac surgery operations (mainly coronary artery bypass grafting) with the beating heart, without aortic cross clamp and whenever it is possible without the use of extracorporeal circulation, because it has been demonstrated that better results can be obtained that those are observed with the traditional techniques of myocardial protection with cardioplegic solutions. This method led us to also consider the possibility to carry out surgery of the mitral valve with continuous coronary perfusion and the beating heart. In the presently work we proposed as objective to expose the form in that we have carried out this surgical technique of myocardial protection in the period understood between January of the 2000 and December of the 2001 in our Institution, as well as to evaluate its security and effectiveness. The results of our study suggest that the mitral valve surgery carried out with the beating heart constitutes an alternative method of myocardial protection, sure and easily reproduce with which satisfactory clinical results can be reached.
