Tratamiento Multicomponente de un Caso de Mutismo Selectivo Multicomponent treatment in a case of selective mutism

2006
This study displays the process of evaluation and treatment of a 6 year old girl who fulfi lls the diagnoses criteria for selective mutism according to the DSM-IV-TR. The treatment includes strategies that surpass the individual setting and considers the institutional setting (family and school). With the purpose of reaching the proposed therapeutic objectives, two differentiated phases were designed. The fi rst one included the transmission of information to parents and personnel of the education centre and instruction and modelling of the desirable and non desirable behavior in the girl. The second one consisted in the treatment of the inhibition of the speech, using a multicomponent treatment that integrated a stimulus fading program, verbal prompting, and positive reinforcement plus a token economy and the symbolic exposure in front of the group-class. Results showed that the spontaneous speech in the class and the speech and participation in games measured as percentage were progressively increasing and staying throughout the short, medium and later follow-ups.
