Clinical Significance of Monitoring Indexes of Oxygen Exchange in Neonatal Respiratory Distress Syndrome

Objective To explore the clinical value of diagnosis and treatment of oxygen exchange in neonatal respiratory distress syndrome(NRDS).Methods Thirty neonates with NRDS were selected as treatment group in neonatal intensive care unit(NICU) from Jan.2003 to Dec.2006,and 23 neonates with infectious pneumonia complicating acute respiratory failure in NICU at the same time were as control group.The changes of respiratory index(RI),oxygenation index(IO),A-aDO2,pa(O2)/pA(O2),pa(O2) in 2 groups were observed in the early period(mechanical ventilation 0-2 h),the middle period(mechanical ventilation 48-72 h)and before ventilation weaning(0.5-2.0 h before ventilation weaning).Results In the early periods,there were significant differences in RI,OI,pa(O2)/pA(O2) and A-aDO2 between 2 groups(Pa0.05),but there was no significant difference in pa(O2) between 2 groups(P0.05).In the middle periods,there were significant differences in RI,OI,pa(O2)/pA(O2) and A-aDO2 between 2 groups(Pa0.05),but there was no significant difference in pa(O2) between 2 groups(P0.05).Before ventilation weaning,there was no significant difference in all criteria between 2 groups(Pa0.05).Conclusion Indexes of oxygen function may become criteria of early diagnosing NRDS,observing effect of treatment and guidance of ventilation weaning.
