Estudio de concordancia entre los resultados de la puntuación de Gleason de biopsias de próstata y los de la prostatectomía radical en pacientes con cáncer de próstata localizado

Introduction: Differences are found between concordance of the Gleason score and the final pathology in prostate biopsies. Objective: To determine the concordance of Gleason scores obtained by prostate biopsy and radical prostatectomy (RP) at a quaternary care center in Bogota D.C., Colombia. Materials and methods: Retrospective study of diagnostic tests performed in a quaternary care hospital in Bogota D.C. The results of the RP and prostatic biopsies were reviewed, and the patients were classified according to D’Amico. The groups were divided by pathologists. Diagnostic concordance was determined by estimating the weighted Kappa coefficient. Results: 180 patients with an average age of 61 years were included. The diagnostic accuracy of the Gleason score given by biopsy was 52%, with 34% superstaging and 14% understaging. The weighted Kappa index between the Gleason score of prostate biopsy and RP was 0.37 (p=0.000), and 0.4 (p=0.000) by groups, with acceptable concordance for the sample. Conclusions: The concordance of prostate biopsy with RP is low, with a tendency to overstaging. This could have a negative impact on the management of patients with localized prostate cancer.
