Study of HIV drug virological failure to First-Line antiretroviral therapy in four cities of Henan

Objective To study the condition of HIV-1 drug resistance among virological failures of first-line antiretroviral therapy(ART) in Henan. Methods A cross-sectional survey was carried out in 4 major epidemic cities from June 2010 to November 2011. Adult patients who experienced virological failure (virus load ≥1 000 copies/mL) with >1 year of first-line antiretroviral treatment were selected for genotype resistance testing. The prevalence and related risk factors were analyzed. Results 2 529 patients were included to analysis in this study, 92.41% of subjects were reported former plasma donors as the route of HIV infection. The CD4 counts and treatment duration were associated with drug resistance, and 63.35% of patients acquired drug resistance. The nucleoside reverse transcriptase inhibitor (NRTI), non-(N)NRTI, and protease inhibitor resistance were found in 48.95%, 60.02%, and 1.47% of subjects, respectively. There was a comparatively high prevalence of an estimated resistance to EFV and NVP, which reached 60.02%, and of the 38.87%、35.31%、36.10%、35.03% and 27.52% subjects in this cohort had an estimated resistance against 3TC, AZT, D4T, DDI and TDF. Conclusions Former plasma donors are also occupy an important position in the populations who have experienced virological failure to first-line ART in Henan during 2010-2011, timely virological surveillance, effective therapy adjustment and more feasibility access to new kinds of drugs were needed to be ensured in Henan.
