Risk factors and treatment for biliary infection of diagnosis and therapeutic ERCP

Objective To investigate the influence of bile duct obstructive disease and position of bile duct obstruction on biliary infection,and analyze the analysis of etiology of bile duct infection and susceptibility.Method Biliary infection of complication of diagnosis and therapeutic ERCP were respectively reviewed in 587 cases in our hospital between January 1990-December 2003,all cases were divided into biliary infection group and biliary non-infection group,each group were divided into 4 sub groups:malignant obstruction,bile duct stone,benignity obstruction and normal bile duct sub group,the difference of biliary infection were compared among different groups.Results 120 cases with biliary infection were diagnosed in post–ERCP,malignant bile duct obstruction had the highest incidence of biliary infection,Porta hepatis obstruction had the highest incidence of biliary infection among another 4 sub groups,there was significant difference(P0.01).In 44 cases infected blood cultures were positive,Pseudomonas aeruginosa,Escherichia coli and Enterobacter cloacae were the main etiology(79.5%).Conclusion The malignant bile duct obstruction and porta hepatis obstruction is the main risk factor for postoperative bile duct infection,It is two important component element for ensuring biliary drainage in post-ERCP and using effectively antibiotic in earlier period infection.
