Confirmación de la metodología diagnóstica diseñada para evaluar la dinámica cardiaca aguda durante 15 horas

Introduction. The calculation of the occupancy space of a chaotic cardiac attractor in a phase space has allowed establishing mathematical and geometric differences between a normal cardiac dynamic of an acute one in patients of the Intensive Care Unit. Objective: To confirm the clinical applicability of the diagnostic methodology based on the theory of dynamic systems that evaluates the normal and pathological state of the cardiac dynamics in 15 hours. Methodology. 25 Holter and/or continuous electrocardiographic records were taken, normal and with acute pathology, evaluated during 21 and 15 hours. From the heart rate values, attractors were generated. Occupancy spaces and the fractal dimension of attractors were constructed based on the theory of dynamic systems, and the previously established parameters were applied. A comparison of the results obtained in 21 and 15 hours was carried out, with conventional diagnosis. To confirm the applicability and clinical reproducibility of the methodology, sensitivity and specificity measurements were performed with respect to Gold Standard. Result.The spatial occupation in 15 hours for the normal dynamics using Kg grid varied between 87 and 133, and with Kp grid between 235 and 301. In contrast, for the acute dynamics using grid Kg, values between 16 and 41, and with Kp grid between 47 and 72. On the other hand, fractal dimension of the normal attractors varied between 1.0243 and 1.5163 for the highs between 0.8765 and 1.9739. Sensitivity and specificity of 100% were obtained. Conclusion. the methodology achieves in a shorter time to establish normal and adverse cases through the spaces occupied by the attractors, findings that are very useful for scenarios such as the ICU.
