Diagnosis and surgical treatment of the coracoid fracture

2009
Objective To discuss the diagnosis and surgical treatment of different types of coracoid fracture. Methods From August 2006 to August 2008,7 cases of coracoid fracture were treated in our department. According to the Eyres classification, there were 1 case of Type ⅠB,2 cases of Type ⅢB,3 cases of Type ⅤA,and 1 case of Type ⅤB. Five cases (3 cases of Type Ⅴ A,1 case of Type ⅤB and 1 case of Type Ⅲ B) were treated with open reduction and internal fixation with percutaneous lag screw through the coracoid base to the scapular neck. All the aeromion-clavicular joint dislocations were reduced with hooked plates. Results All the patients received follow-up for a mean of 14. 1(6 to 24) months. The mean Constant-Mudey score for the surgical group was 90 points. The mean Herscovici score for the surgical group was 14.4 points. Conclusions The fracture of the coracoid usually happens with the upper glenoid fracture and is accompanied by the ipsilateral acromion-clavicular joint dislocation. Surgery is strongly rec-ommended when the whole shoulder girdle is in an unstable condition. Key words: Acromioclavicular joint;  Dislocation;  Fracture;  Scapula;  Fracture fixation, internal
