Protastectomía Radical Laparoscópica: Primeros 50 casos.Nuestra propuesta técnica. Abordaje extraperitoneal.

The Laparoscopic Radical Prostatectomy (LRP) has always relied on an increasing number of followers in the international urological community since it was described and spread by means of the studies carried out France Montsouris Institute in 1998 by Dr. Guillonneau and Dr. Vallancien. This new technique contributes advantages of the laparoscopy not only for the patient but for the surgeon as well. Almost all the urologist who perform the LPR in Europe and USA, approach the Prostate in a transperitoneal way and point out variables in the first surgical time as a difference. Being an example of this, the technique of a primary access to the Seminal Vesicle, proposed by Drs. Guillonneau and Vallancien as well as the one with a primary access to the Retzius' space, proposed by German authors such as Dr. Rassweiler. As a result of our first experience with 12 transperitoneal LRP, since the first December of 2000, and in the search of reproducing the conventional retropubic approach using laparoscopy, we are introducing in this study our extraperitoneal technique, after having evaluated 38 cases, since the ninth November of 2001. The benefits of not entering the abdominal cavity have been shared in this new approach. A post-surgical recovery improvement, earlier resumption of activities and earlier intake, and hospital discharge within the first 24 to 48 hours, have been reported as a result.
