Lenstar Optik Biyometrinin Farklı Katarakt Tiplerinde Performansının Değerlendirilmesi

Amac: Lenstar goze temas gerektirmeyen ve kullanimi oldukca kolay olan nisbeten yeni bir optik biyo- metridir. Ancak cihaz ile tum katarakt hastalarindan olcum almak mumkun olmamaktadir. Bu calismanin amaci Lenstar ile olcum alinamayan katarakt tiplerini belirlemek ve olcum alinabilen hastalarda cihazin hedef refraksiyondan sapmasini degerlendirmektir. Gerec ve Yontemler: Bu calismaya Bozok Universitesi Arastirma ve Uygulama Hastanesinde katarakt ameliyati olan ardisik 339 hastanin 398 gozu dahil edildi. Hastalarin detayli oftalmolojik muayenelerinin ardindan katarakt tipleri kaydedildi. Tum olgularda goz ici lens gucunun hesaplanmasi icin oncelikli ola- rak Lenstar ile biyometri yapildi, olcum alinamayanlarda ultrasonik biyometri kullanildi. Ameliyat sonrasi 1.ayda hedef refraksiyondan  sapma hesaplandi. Bulgular: Olgularin 157’si kadin, 182’si erkekti ve yas ortalamasi 66,94±11,37 idi. Lenstar ile gozlerin 136 tanesinde (%34,1) olcum alinamadi. Olcum alinamayan gozlerin %54,4’unun (n=74) katarakt tipi matur, hipermur ve rubra iken, %22,7’si (n=31) yogun arka subkapsuler katarakt, %17,6’si (n=24) yogun nukleer katarakt ve %5,1’i (n=7) arka polar katarakt tipine sahipti. Olcum alinamayan kisilerin ameliyat oncesi gorme duzeyi 108 gozde (%79,4) 0,05 ve altinda iken, 85 gozde (%62,5) el hareketi-1 metreden parmak sayma seviyesindeydi. Lenstar optik biyometri ile hedef refraksiyondan sapma miktari -0,23±0,45 D idi. Sonuc: Lenstar optik biyometri ile olcum alinamayanlar, ozellikle matur ve yogun arkasubkapsuler ka- tarakti olan ve gorme duzeyi oldukca dusuk hastalardir. Olcum alinabilen gozlerde ise refraktif sonuclar mukemmele yakindir. Anahtar kelimeler: Goz ici Lens; Lenstar; Katarakt; Okuler refraksiyon. ABSTRACT Objective: Lenstar is a comparatively new optical biometer which is very easy to handle and do not necessitate eye contact. However, it is not possible to perform measurements in all cataractous patients with this device. The aim of this study is to determine the cataract types those are unmeasurable with Lenstar and to assess the deviation from target refraction in patients that Lenstar could perform measurement. Material and Methods: Three hundred ninety eight eyes of 339 patients who underwent cataract surgery successively at Bozok University Research and Practice Hospital were included in this study. After the complete ophthalmological examination, cataract types of the patients were recorded. To calculate the intraocular lens power, Lenstar was used primarily in all cases, whereas ultrasonic biometer was used in unmeasurable cases with Lenstar. Deviation from target refraction was calculated one month after the surgery. Results: There were 157 women and 182 men and mean age of the patients was 66.94±11.37 years. Measurement could not be performed with Lenstar in 136 eyes (34.1%). Eyes with measurement failure had mature, hypermature and rubra cataract in 54.4% (n=74), dense nuclear cataract in 17.6% (n=24) and posterior subcapsular cataract in 5.1% (n=7). One hundred eight (79.4%) eyes with measurement failure had visual acuity of 0.05 or less, whereas 85 of them (62.5%) had visual acuity between hand motion to 1 meter finger count. The deviation from target refraction with Lenstar optical biometer was -0.23±0.45 D. Conclusion: The patients unmeasurable with Lenstar were the ones having especially mature and dense posterior subcapsular cataract and low visual acuity. Refractive outcomes in eyes measurable by Lenstar were excellent. Keywords: Intraocular Lens; Lenstar; Cataract; Ocular refraction.
