Effect of respiratory support for the treatment of neonatal respiratory distress syndrome and risk factors analysis

Objective: To discuss the therapeutic effect of eonatal respiratory distress syndrome with different respiratory support method for the treatment of neonatal respiratory distress syndrome effect and to analyze the risk factors that cause neonatal death. Methods: Clinical data were retrospective analyzed in 64 cases with neonatal respiratory distress syndrome. 34 cases of those patients were treated with nasal intermittent positive pressure ventilation( NIPPV),and 30 cases with nasal continuous positive airway pressure( NCPAP). Blood gas analysis and the incidences of complications and rate of converted to mechanical ventilation were observed and the major risk factors causing neonatal death were also analyzed before and after treatment. Results:( 1) The comparison of blood gas analysis between the children who were treated with NIPPV and those who were treated with NCPAP: pH and PO2 of the NIPPN group were significantly higher than those of the NCPAP group at 1 h and 12 h after treatment,while PCO2 and FiO2of NIPPN group were significantly lower than those of the NCPAP group( P 0. 05).( 2) The comparsion of two groups in the incidences of complications and the rate that were converted to mechanical ventilation: the incidences of complications and the rate that were converted to mechanical ventilationb of NIPPV group were significantly lower than the NCPAP group( P 0. 05).( 3) The analysis of neonatal death risk factor:birth weight and the highest A-aDO2 were the only two independent risk factors for neonatal death,the regression coefficients were 0. 003 and 0. 011,and OR were 0. 992 and 1. 073 respectively. Conclusion: NIPPV is better for treating neonatal respiratory distress. Birth weight and the highest A-aDO2 are the only two independent risk factors for neonatal death.
