TRATAMIENTO PRESERVADOR DEL CÁNCER DE LA MAMA EXPERIENCIA EN CENTRO CLÍNICO DE ESTEREOTAXIA

SUMMARY OBJECTIVES: The objective of this work is submitting the experience of our institution in the surgical treatment of breast cancer and analyzing the conservative surgery treatment in terms of global survival, free disease survival, recurrences and the esthetic results. METHODS: During December 1995 and December 2005, we treated 436 patients with breast cancer. A 58.48 % (255/436) underwent conservative surgery procedure. The decision to preserve the breast was determined by obtaining satisfactory esthetic result with free margins. Patients with no epithelial tumors were excluded, and patients with bilateral disease also. Most of them received treatment with post operatory radiotherapy and according to the institutional criterion adjuvant chemotherapy. RESULTS: Age group between 30 and 40 years had a lower survival, and 80 years and older had longest survival. The patients with tumors sizes 3 cm had a shorter survival. The patients with non palpable lesions size or extension has evidenced of better survival. Ductal in situ carcinoma and the microscopic ductal in situ carcinoma evidence better survival than lobular infiltrating carcinomas. The esthetic result was satisfactory. 5.4 % of relapses and 2.1 % corresponded to breast. The global survival was 94.4 % and free from disease survival 95.5 %. CONCLUSION: Conservative treatment is an alternative in the surgical treatment of breast cancer. In the case of Institutions that are devoted to breast pathology, patients are detected at an early stage of the disease and this enables better results to be obtained.
