Interrater-Reliabilität der deutschen Autar-DVT (Deep Vein Thrombosis)-Skala zur Einschätzung des Thromboserisikos

The exact and precise assessment of the thrombosis risk is of great importance in practice and research. If thrombosisrisks are identified early on, preventive measures can be taken early enough in order to reduce the risk of adeep vein thrombosis (DVT) as well as of a pulmonary embolism. The assessment should be undertaken by meansof a risk scale. The aim of the study was to investigate the German version of the Autar-DVT-Scale regarding itsinterrater reliability and observation conformity in acute clinical practice. The evaluation of the test-theoreticalquality criteria within the framework of a cross-sectional study was carried out with a sample of 181 patients and acorrelative and multicentre design.The Kappa values of the item scores ranged between κ = 0.00 and κ = 1.00. The highest interrater reliabilities weremeasured with 15 items (κ = 0.862-1.000). The lowest Kappa values (κ = -0.006-0.392) were measured with theitems of “pelvis injury”, “abdominal” and “neurosurgical”. In all specific departments involved, the Kappa values ofthe overall scores ranged from “considerable” (κ = 0.63) to “almost perfect” (κ = 0.97) observation conformities(78.3-97.5%; ICC = 0.806-0.994). The results prove an intermediateto high interrater reliability and observation conformity ofthe German Autar Deep Vein Thrombosis Scale for the assessmentof the thrombosis risk in patients by carers. Further investigationson the quality of the scale are recommended.
