Acute Lumbar Radiculopathy

Lumbar radiculopathy is a pathologic process affecting a spinal nerve root. The most common causes are herniated discs and spinal stenosis. A thorough history and physical examination will demonstrate sensory deficits in specific lumbar dermatomes and possibly weakness in the corresponding myotome. Treatment protocols vary depending on the severity of the pain and/or weakness. Initial conservative management includes medications, spinal manipulation, physical therapy, modalities, and activity modification. Interventional spinal procedures may be indicated in moderate to severe painful radiculopathies, and surgery is reserved for progressive neurologic decline, new bowel and bladder dysfunction(s), and intractable pain.
