INFECÇÃO E COLONIZAÇÃO POR BACTÉRIA PRODUTORA DE CARBAPENEMASE EM PACIENTES DE UM HOSPITAL TERCIÁRIO: CARACTERIZAÇÃO CLÍNICA E EPIDEMIOLÓGICA INFECÇÃO E COLONIZAÇÃO POR BACTÉRIA PRODUTORA DE CARBAPENEMASE EM PACIENTES DE UM HOSPITAL TERCIÁRIO: CARACTERIZAÇÃO CLÍNICA E EPIDEMIOLÓGICA

2015
Background: With the emergence of carbapenemase-producing Enterobacteriacea (CPE), the treatment of infections acquired in the hospital environment has become even more challenging, with consequent shortage of treatment options. Objective: characterize clinically and epidemiologically patients in which CPE was isolated. Methods: Patients who had positive culture for CPE in surveillance cultures or clinical sample between January and July / 2014 were studied. Several variables were analyzed in order to detect the most frequent characteristics. Results: A total of 63 patients were evaluated, with mean age of 52 years. Forty-four patients were admitted to Hospital because of non-infectious diseases. The most frequently found comorbidities were cardiovascular complications. Thirty-seven (58, 7%) patients proceed from other health care facilities. The most common units where CPE was detected were: adult intensive care units (66,7%), internal medicine (11%), general surgery (6,3%), cardiac intensive care unit (3,2%) and bone marrow transplantation (3,2%). From all patients, 12 (19%) had CPE isolated from clinical samples. Considering the hospital length, only 12 (19%) patients had the detection after 30 days of admission. Four (6,3%) patients died, all not related to CPE infection. Conclusions: In a tertiary hospital, the vast majority of isolations occurred in intensive care units and within 30 days of admission. Many patients already came from other health facilities and had previous chronical diseases. In the few deaths that occurred, none was linked to the CPE infection.
