Hipernefroma con trombo tumoral en vena cava inferior y aurícula derecha: conducción de la circulación extracorpórea

RESUMEN   Los tumores renales malignos con trombos tumorales que involucran a la vena cava inferior y a la auricula derecha son raros. Presentamos un caso operado exitosamente de un hipernefroma renal, con trombosis tumoral de vena cava inferior Grado IV, que progresaba hasta la auricula derecha y la valvula tricuspide, con el empleo de circulacion extracorporea. El uso de tecnicas como la hipotermia moderada, el drenaje venoso asistido por vacio y la recuperacion de la sangre del campo quirurgico aseguraron, en primer lugar, alcanzar los flujos teoricos calculados de perfusion, brindar una adecuada proteccion cerebral y esplacnica a la vez que permitieron, el establecimiento de condiciones optimas para la reseccion total de la masa tumoral renal y el trombo tumoral presente en la vena cava inferior y la auricula derecha. El paciente tuvo una evolucion postoperatoria satisfactoria y fue dado de alta para el hogar despues de 10 dias de operado. ABSTRACT Malignant renal tumors with tumor thrombus that involve to the inferior vena cava and to the right auricle are uncommon. We present a case operated successfully of a renal hypernephroma, with inferior vena cava tumoral thrombus Grade IV, which progressed until the right auricle and the tricuspid valve, with the employment of extracorporeal circulation. The use of techniques as the moderate hypothermia, vacuum assisted venous drainage and the recovery of the blood from the surgical field assures, in the first place, to reach the calculated theoretical flows of perfusion, to offer an appropriate cerebral and organ protection and at the same time they allows, the establishment of good conditions for the total resection of the renal tumoral mass and the tumoral thrombus present in inferior vena cava and right auricle. The patient had a satisfactory postoperative evolution and he was discharge for the home after 10 days of being operated.   Key words: Hypernephroma, extracorporeal circulation, vacuum assisted venous drainage, tumoral thrombosis, inferior vena cava.
