105: COVID Busters: A Multidisciplinary Team for Rapid COVID-19 System-Wide Process Implementation

INTRODUCTION: Given the unpredictability of a global pandemic and its rapid evolution, in March 2020 our 200- bed children's hospital began surge preparations, which would require rapid review and implementation across the system Our institution is the only quaternary children's hospital within an organization of 26 hospitals across the state and 47 nationwide We describe our process for rapid implementation of evolving guidelines in our system METHODS: A group of leaders within pediatric hospital medicine, critical care, neonatology, nursing, pharmacy and respiratory therapy organized a COVID-19 pediatric surge response team with executive leadership support This multidisciplinary team became known as the ?COVID Busters ? The group was tasked with ongoing review of disease related updates, strategic pediatric pandemic response and surge planning within our system, with rapid process implementation to meet evolving needs Educational material was also produced for both inpatient needs and those of the pediatric medical community All processes, guidelines, and algorithms were based upon evidence-based medicine, best practices and were subject to rigorous peer review within the organization's scientific committee prior to implementation across the system Rapid communication of changes was available via an electronic application for mobile devices Regular interval reviews were performed on each process and re-submitted for peer review when indicated RESULTS: Over 15 clinical algorithms have been created, thus far These algorithms have been used for prompt diagnosis, treatment, resource allocation and stewardship in critically ill children with COVID-19 infection, including multisystem inflammatory syndrome in children Educational initiatives included multidisciplinary simulations and instructional videos, as well as livestream and expert interview question/answer sessions Emerging issues of mental health, family stressors, and return to school issues have also been addressed We have treated >30 hospitalized pediatric patients with COVID-19 infection at our children's hospital with no mortalities to date CONCLUSIONS: Creation of a highly structured multidisciplinary team facilitated the delivery of high-quality care for children affected by the COVID-19 pandemic across a large health care system
