Applied study of bronchoscopic lung volume reduction in emphysema

Objective:To investigate the possibility,treatment effectiveness and complications of bronchoscopic lung volume reduction by airway membrana tectoria cochleae stents made in China in goat model of heterogeneous emphysema.Methods:4 female goats were used for localized papain instillations to generate heterogeneous emphysema.1~2 airway membrana tectoria cochleae stentses were embedded in the different segmental bronchus of emphysema regions(anterior segment or posterior segment of superior lobe of right lung)by bronchofibroscope.1 day before and 6 weeks after the surgery,lung function,blood gas analysis and CT scans were detected.The pulmonary infection and other complication were deteceted.Results:7 valves were successfully sent out,while 1 was expectorated.TLC,FRC,RV decreased after 6 weekst(P0.05).RV/TLC(%) was decreased from 42.55 % to 20.37 %.PaO2 increased from(70.50±1.85)mmHg to(81.25±1.11)mmHg(P0.05).PaCO2 decreased slightly,which has no significant(P0.05).Atelectasis was confirmed by CT scan and the specimen of lung tissue.Only 1 goat has the early symptoms of lung infection.no pneumothorax and pulmonary abscess happened in all animals.Conclusion:Bronchoscopic lung volume reduction by one-way valve made in China to treat sheep model of heterogeneous emphysema is effectiveness.Minor trauma.safety and easy operation.
