Terapéutica de las Leishmaniasis Americanas. Nuevas recomendaciones

Durante mas de 30 anos el tratamiento de las diferentes formas de leishmaniasis tuvo como medicamento de eleccion los antimoniales pentavalentes (Glucantime® y Pentostam®), las dificultades asociadas al tratamiento, en ocasiones fatales, creo la necesidad de desarrollar nuevas alternativas terapeuticas tanto locales como sistemicas. En el 2010, durante la ultima reunion del Comite de Expertos de la OMS, dichas opciones fueron recogidas y adoptadas, se dejo en manos del medico tratante la decision de que esquema terapeutico utilizar en su paciente, de acuerdo a las caracteristicas clinicas de la enfermedad,la especie del parasito, el estado inmune del paciente y la disponibilidad de medicamentos, hasta en ocasiones no administrar ningun medicamento, como puede ser el caso de lesiones en proceso de cicatrizacion, sea en forma espontanea o por tratamientos empiricos y aun en pacientes con leishmaniasis atipica. En este articulo se describen las diferentes opciones de tratamiento sistemico y local, como tambien se orienta al clinico al tratamiento especifico segun la especie del parasito. New Recommendations Abstract: For more than 30 years, the standard for treating the different forms of leishmaniasis were pentavalent antimonials (Glucantime and pentostam), the pitfalls associated to the treatment, sometimes leading to death, created the need to develop new therapeutic alternatives both local and systemic. In 2010, during the last meeting of the WHO executive board, those alternatives were gathered and implemented; they left to the clinician the decision of the treatment scheme to be used with each patient, according to the clinical characteristics of the disease, the parasite species, immune status of the patient and the availability of medication. In some cases they could decide not to administer any medication, as for example in scarring lesions, where the healing was spontaneous, or by empirical treatments, or in patients with atypical leishmaniasis. In this article the different treatments, local and systemic, will be described, and the clinician will be oriented towards a specific treatment according to the parasite species.
