An unusual cause of ‘dextrocardia’

A previously fit and well 37 year-old man was diagnosed with ESC grade 1 arterial hypertension. His personal medical history was only remarkable for longstanding chest wall asymmetry, never previously investigated. There was no relevant family history. An electrocardiogram was Sokolow-Lyon voltage criteria positive for left ventricular hypertrophy (LVH) ( Figure 1A ). Chest radiograph revealed apparent dextrocardia ( Figure 1B ). Cardiac magnetic resonance (CMR) was performed to: (i) confirm dextrocardia and exclude concomitant cardiac congenital abnormalities, (ii) confirm the presumptive diagnosis of hypertensive LVH and (iii) investigate for underlying causes of young-onset hypertension.
