Abstract 129: Implications of Incorrect Documentation and Coding of Troponin Elevation

2019
Background: Though clinicians provide appropriate care by clinically differentiating between acute myocardial infarction (AMI) and non-thrombotic cause of troponin elevation (NTTE), documentation and appropriate coding of the same can have implications on hospital level outcome metrics, accreditation and public reporting for quality of care. We aim to evaluate the implications of incorrect coding of NTTE. Methods: This is a single-center retrospective analysis of 465 patients discharged with a principle ICD-10 diagnosis code of I21.4 or I21.A1 between January 2017 to December 2018. All charts were reviewed, including admission notes, cardiology consult notes and discharge summaries relevant to obtain discharge medications restricted to dual antiplatelet therapy. Of the total 465 patients that were reviewed, NTTE were separated from patients with AMI based on The Fourth Universal Definition of Myocardial Infarction. Results: Of the total 465 patients who were coded as Non-ST Elevation MI (NSTEMI), 54.41% o...
