113-P: SEARCHING UNRELATED DONORS FOR HPC TRANSPLANTATION. EXPERIENCE IN A PUBLIC PEDIATRIC TRANSPLANT CENTER IN BUENOS AIRES, ARGENTINA

2012
Aim Prompt identification of matched unrelated donors is critical for patients in need of HPC transplant. In general, Argentinean population shows Hispanic and Caucasian HLA alleles well represented in international registries. However, some patients show Native South American alleles, underrepresented in the donor pool. This is a potential disadvantage for our patients that might take longer to find a donor. Our aim was to describe HPC donor search in our population in order to estimate time and success of the search. Methods We carried out a retrospective evaluation of 48 unrelated donor (UD) searches started in our Hospital from January 2010 to December 2011. Success was defined as one or more bone marrow donor 9 - 10/10, 8/8 or UCB unit 5 - 6/6 (with more than 3 × 10 7  NC/kg) considering high resolution results. Time to find a donor is presented as median (range). Unavailability donor rate was defined as number of CT request not performed over total CT request (N = 242). NMDP HaploStats was used to compare haplotypes present in patients for which we found or not a donor. Results Total success was 73 %. Donors were found mainly from NMDP and ZKRD followed by Brazil and Argentina registries. Number of donors per patient was: 13 one donor, 14 two donors and 8 three donors. Time spent in the search process was 3 (1-15) months. Thirteen patients did not found a donor. The proportion of unavailable donors was 40% (97/242). HaploStats analysis showed that for patients without a donor 38% have two inferred haplotypes present, 54% one haplotype and 8% have none haplotype. Patients that found a donor showed 83% and 17% with two and one haplotypes respectively. Conclusions This initial analysis allowed us to estimate the proportion of our patients that found a donor and the time to find it. Further knowledge of reasons for not finding a donor will help us to improve search strategies.
