Long-term effect of periurethral Macroplastique injection for treatment of stress urinary incontinence

Purpose:To observe the short term and long term effects of periurethral Macroplastique injection for treatment of female stress urinary incontinence.Methods:Twelve women with stress urinary incontinence were treated with transurethral injection of Macroplastique beneath urethra mucosa 0.5 - 1.0 cm distal to the bladder neck under cystourethroscopic control.The short and long term outcome were assessed by clinical outcome and/or urodynamics and transrectal ultrasound scanning at 3 months and 47 months after injection.Results:The mean 3.5 pads per day were required and Marshall Marchetti test were all positive in our series.The mean length of urethra was 3.0 cm.The mean maximum urethral pressure was 3.73 kPa (n=8).The mean 4.8 ml of Macroplastique was injected.The short term efficacy rate was 77.8 % at 3 months post operation.The long term efficacy rate was 55.6 % at mean 47 months of following up.Those with less or no bladder neck mobility had better response,and those in which the silicone elastomer cycled the bladder neck did also.Conclusions:Periurethral injecting Macroplastique for stress urinary incontinence produced a good short term and long term results. This method was a relatively simple and minimally invasive and had better short term and long term efficacy for stress urinary incontinence with less bladder neck mobility.
