Clinical study on viscocanalostomy of schlemm's canal for primary open-angle glaucoma

Objective To evaluate the efficacy and safety of viscocanalostomy compared with trabeculectomy for treating far-advanced primary open-angle glaucoma.Methods Sixty eight eyes of 68 patients with primary far-advanced open-angle glaucoma scheduled for glaucoma surgery were enrolled and allotted to group A (viscocanalostomy,33 patients) and group B (trabeculectomy,35 patients) randomly in the study.Surgeries were performed by one surgeon.We recorded intraocular pressure (IOP),complications,visual acuity,and eye examinations as routine to monitor any progression of glaucoma preoperatively,at 7 d,14 d and 1,3,6,12 months postoperatively.Results There was no statistical difference in controlling the IOP between the 2 procedures.There was also no statistical difference in the visual acuity between the 2 groups.For complications,there were 2 cases of anterior chamber inflammatory reaction,1 case of hyphema in group A.But in group B,there were 7 cases of anterior chamber shallowness,3 cases of ciliary body or choroidal detachment,2 cases of hyphema,3 cases of concurrent cataract and all patients had anterior chamber inflammatory reaction.The surgery achievement ratio were 66.67% (22/33) and 68.5% (24/35) for group A and B respectively and there was no statistical difference.Conclusion The two groups have similar surgery achievement ratio.For complications,group A has less complication ratio and the severity is mild.So,group A is safer and more effective.
