DCA211 failure analysis report

This report contains the findings, conclusions, and recommendations of a committee that was commissioned to investigate the events leading to the burnout of the main power connection to the test stand in dipole magnet DCA211 during tests at Brookhaven National Laboratory (BNL) on April 8, 1992. The report contains an executive summary of the significant findings, conclusions, and recommendations; this summary is followed by a more detailed compilation of reports, calculations, and data that were used by the committee to reach our conclusions.
