A 72-year-old woman presenting with several episodes of decompensated heart failure was proven to have features of severe mitral regurgitation due to a combination of ischemic cardiomyopathy resulting in adverse LV remodeling and mild rheumatic heart disease. She underwent successful edge-to-edge repair with the MitraClip system.
