A CASE OF GIANT PARATHYROID ADENOMA

A 52-year-old female was admitted to our hospital, because of heavy lumbago for which, she was not able to walk. Blood Ca level was 7.4 mEq/l and c-PTH, 19.6 ng/ml. Blood ALP level was also high. X-ray film of the head showed salt and pepper appearance and that of spine showed ragger jersey appearance. CT scan of the neck and chest showed an oval and smooth tumor located from the upper mediastinum to the neck. The patient was diagnosed as hyperparathyroidism due to the tumor. The tumor was excised from the neck wound without mediastinotomy. It was a parathyroid tumor, 28 g in weight and 6×4 cm in size, locating in the site which coincided with the location shown in the CT film. Pathologically it was diagnosed as parathyroid adenoma. After operation, heavy lumbago disappeared and the bone condition was getting better. Blood PTH and Ca level fell to the normal range. As of one year after operation, there is no abnormal lesion on her chest X-ray film, and Ca and PTH level are within normal limits.
