Perfusion defects on standard lung scintigraphy can be due to many causes, though typically from occlusion or partial obstruction of the pulmonary arteries. This case presentation highlights that outflow obstruction due to large pulmonary venous disease may result in perfusion abnormalities, and should be considered in the differential when a clinician is presented with an abnormal lung ventilation-perfusion scan.
