Plasminogen activator inhibitor-1 (PAI-1), the primary inhibitor of tissue- and urokinase-type plasminogen activators is considered a critical regulator of the fibrinolytic system. Impaired fibrinolytic function as assessed by an elevated plasma concentration of PAI-1 is a marker for atherothrombotic events. PAI-1 plasma concentration is dependent on many environmental and genetic factors such as body mass index, plasma insulin and triglycerides concentration and 4G/5G polymorphism in the promoter region of PAI-1 gene. This review describes clinical implications of PAI-1 plasma concentration, especially in cardiovascular disorders.
