Hernie de Claudius Amyand : à propos de deux observations à l’hôpital national de Zinder

La hernie de Claudius Amyand (HCA) est definie par la presence d’un appendice dans un orifice herniaire de la paroi abdominale. Le but de cette etude etait de decrire la prise en charge chirurgicale de deux cas de hernies de Claudius Amyand a l’hopital national de Zinder (HNZ) a la lumiere de la litterature. Il s’agissait de deux patients âges de 26 et 37 ans, admis en urgence dans un tableau de hernie inguino-scrotale etranglee. L’intervention chirurgicale avait permis de decouvrir l’appendice dans le sac herniaire. Pour le premier patient,  l’appendice etait normal et pour le 2eme patient, il etait inflammatoire. Une appendicectomie et une cure de la hernie selon la technique de Bassini avaient ete realisees dans les 2 cas. Les suites operatoires etaient simples. La HCA est une entite rare dont le diagnostic est souvent peroperatoire. Son traitement est de plus en plus codifie tenant compte des problemes de contamination. L’appendicectomie et la herniorraphie donnent de bons resultats chez des patients sans peritonite ou sepsis local. Amyand's hernia is a condition in which the vermiform appendix is found within an abdominal hernia sac. The aim of this study was to  describe the surgical management of two cases of Amyand's hernia at the Zinder National Hospital in the light of the literature. Two  patients of 26 and 37 years with a previously reducible groin hernia were admitted to the emergency department with a painful, non-reducible groin mass. The diagnosis of a strangulated inguinal hernia was made. During the surgical procedure, the vermiform appendix was found in the hernia sac. The appendix was normal in one case and iinflammatory in the second. Patients were   subsequently treated with an appendectomy and a Bassini hernia repair. The postoperative course was  uneventful. Amyand’s hernia (AH) is a rare entity often diagnosed intraoperatively. The treatment of AH is better codified taking into account the problems of contamination associated with  appendectomy and hernia repair. Appendectomy and herniorrhaphy give good results in patients without sepsis or peritonitis.
