The principles for the treatment of intraarticular calcaneal fractures include anatomic reduction of the posterior articular facet, retrieve the width, length and height of the calcaneus. The extended lateral approach in L has won wide popularity for fi xation of intra-articular fractures of the calcaneus and proved excellent exposure, providing access for manipulation and fi xation of fractures. However, Sanders and Benirschke reported complications up to 16%. Patients may develop soft tissue complications, such as fl ap necrosis and infection of the surgical wound. In addition some patients may require amputation because of the failure of the management of wound infection. Other complications include postraumatic subtalar arthritis and pinched lateral peroneal sheath. Often subtalar arthrodesis requires decompression and relieve symptoms.
