Case Study of Maintaining SOAP Pharmaceutical Care Records

OBJECTIVE:To discuss the how's on writing and maintaining SOAP-type pharmaceutical care records in clinical practice.METHODS:An analysis was made on how to write and maintain SOAP-type pharmaceutical care records through case studies of the practice adopted by the infection department of our hospital.RESULT CONCLUSIONS:SOAP is a proven protocol suitable for writing and maintaining pharmaceutical care records,which can not only record various problems occurred in clinical cases but also materialize the concept of pharmaceutical care for patients' safe and effective use of drugs.
