A plan to integrate vaccination into the pediatric service of a primary health care center in the Catalan Institute of Health was intended to demonstrate that a single record system and follow-up of the pediatric population are measures that in themselves increase the extrinsic efficacy of vaccination. The integration was effected in early 1985. Previously vaccination was performed exclusively by the municipal health services. The vaccination program was attended by 2 nurses who offered vaccinations Monday through Friday by appointment. The registration system includes 2 records 1 in which childrens alphabetically ordered records include data on their vaccination status and 1 in which dates of the next vaccination are arranged in chronological order. Parents are informed by telephone or mail of the next vaccination appointment. The number of children vaccinated the reasons for missed appointments the consumption of vaccine and the need for resupply are evaluated each month. The efficacy of the new vaccination program was evaluated by the number of vaccine does administered before and after the integration and during March-July 1984 and 1985. The municipal health services administered 459 doses of vaccine in March-July 1984 while in March-July 1985 the municipal services administered 236 doses and the primary health care center administered 2034. The crude increase in 1 year was 495%. The primary health care center was responsible for 90% of the 2270 vaccine doses administered in 1985. The results indicated an increased degree of protection for the children of the area. The vaccination program has added a previously lacking preventive focus to the primary health care centers activities.
