Age-Related Macular Degeneration in Sawanpracharak Hospital

Objective : To assess the prevalence and risk factors of age-related macular degeneration (AMD) and to assess the sensitivity and specificity of AMD screening tests(Amsler grid, Low contrast Amsler grid and reading test). Setting : Department of Ophthalmology, Sawanpracharak Hospital, Nakhon Sawan. Design : Prospective Descriptive Research Subjects : All patients 50 years of age or older who were presented at Ophthalmology Out-Patient Unit and agreed to participate in the research between March 3 rd , 2008 to March 8 th ,2008. Method : Patients underwent interview, screening test, and clinical examination. The following demographic data were recorded in AMD questionnaire: age, sex, and underlying disease. Each patient was screened with different screening methods: Amsler grid, Low contrast Amsler grid or reading test. All patient visual acuity acuity and fundus were examined. Screening results, visual acuity and fundoscopic examination results are also recorded in AMD questionnaires. The data were analyzed using t-test, Fisher’s Exact test, ANOVA and Chi- square test with statistical significance at P 80, 6.7%. 50.0% of patients were blind and 50.0-83.3% had low vision. No statistic significance in sex, age, history of hypertension and history of previous cataract surgery between AMD patients and none AMD patients. The sensitivity of Amsler grid cannot be assessed but the specificity was 72.9%. In Low contrast Amsler grid the sensitivity was 50.0% and the specificity was 78.2%. In reading test the sensitivity cannot be assessed but the specificity was 46.0%. Conclusion : The prevalence of AMD was not different from other studies in Asian country; however, lowever, lower than developed country. At Sawanpracharak hospital, the prevalence was 1.2% and the highest prevalence was in patients older than eighty years old. Patients with high blood pressure and previous history of cataract surgery might be at greater risk of AMD. Most patients presented with late AMD with either blind or low vision. Amsler grid and low contrast Amsler grid helped detect the abnormality of macular disease though the sensitivity and specificity cannot be assessed due to limited numbers of samples.
