Возможности прогноза глубоких инфекционных осложнений хирургического вмешательства при эндопротезировании тазобедренного сустава

2015
The essay represents a retrospective data analysis for 893 patients who have undergone total hip replacement. In order to conduct comprehensive analysis, the examination file which included data involving more than 100 parameters (information on the indigenous and local objective status, findings of the laboratory, radiological and endoscopic methods of examination, parameters of arthroplasty application and other issues), was generated. based on information about the patients, which was derived from the above files, data on the patients who have undergone hip arthroplasty without complications (1 st group 808 patients) and on the patients who, within 12 months after arthroplasty, developed deep infection in the area of surgical intervention (2 nd subgroup 85 patients ) were used as the training matrix when creating the mathematical prognosis and algorithm of prevention of deep suppuration in patients undergoing primary total hip arthroplasty. 21 prognosticallysignificant criterion of suppuration in the postoperative period has been identified. taking the above criteria into account during prospective study (293 clinical cases) allowed to reduce the incidence of deep suppurative complications in the surgical wound significantly, if compared with the frequency in the group of retrospective studies (from 1.7% to 0.7%). The effectiveness of the proposed program was 80%. Refs 17. table 1.
