AKREP SOKMASI NEDENİYLE BAŞVURAN ÇOCUK OLGULARIN DEĞERLENDİRİLMESİ: BİR ÜNİVERSİTE HASTANESİ DENEYİMİ Evaluation of Pediatric Cases That Present with Scorpion Stings: The Experience Of An University Hospital

Amac: Bu calismada akrep tarafindan sokulan cocuklarin demografik ozellikleri, semptom ve bulgulari ve tedavi sonuclari analiz edilerek bu konuda literaturde cocuk hastalarla ilgili kisitli bilgiye katkida bulunulmasi amaclanmistir. Gerec ve yontemler: Akrep sokmasi nedeniyle hastanemize basvuran toplam 37 cocuk geriye donuk incelendi. Olgular klinik siddetlerine gore gruplandirildi. Evrelerine gore klinik ve laboratuvar ozellikleri karsilastirildi. Bulgular: Yas ortalamalari 8±4.6 yil olan 37 olgu retrospektif olarak degerlendirildi. 24’u (%65) erkek, 13’u (%35) kizdi. Olgularin %83,7’si hafif-orta klinik siddete sahipti. Lokal agri (%60) ve lokal eritem(%30) basvuru anindaki en sik bulgulardi. En fazla sokma olayi yaz mevsiminde (%75.6), evde (%81) ve gece saatlerinde (%54) gerceklesmisti. Hicbir olgu da otonomik firtina, kalp yetersizligine bagli veya kalp disi nedenle akciger odemi ve kalp yetersizligi gelismedi. Hicbir olgu mekanik ventilatore baglanmadi ve olmedi. Yogun bakima yatirilan ve antivenom uygulanan siddetli olgularin hicbirinde alerjik reaksiyon gozlenmedi. Sonuc: Bolgemizde akrep sokmasina bagli mortalite orani dusuk olsa da, akrep sokmasi ulkemiz ve bolgemiz icin halen onemli bir saglik sorunudur. Bu nedenle halka oncelikle korunma tedbirleri ogretilmeli ve en kisa surede saglik kuruluslarina basvurmalari saglanmalidir. Anahtar kelimeler: Akrep; Cocuk ;Yogun bakim ABSTRACT Aim: The aim of the study was to evaluate the demographic, clinical characteristics, symptoms, signs and outcomes of children who were bited by scorpion and to contribute to the limited data in the literature about pediatric patients. Material and Methods: A total of 37 children admitted to our hospital due to scorpion bites were examined retrospectively. The cases were grouped according to clinical severity. The clinical and laboratory characteristics were compared according to their stages. Results: Thirty-seven patients with an average age of 8±4.6 years were evaluated retrospectively. It was determined that 24 (65%) were male, and 13 (35%) were female. A total of 83.7% of the patients had mildmoderate clinical severity. Local pain (60%) and local erythema (30%) were the most common symptoms when the patients presented. The stings occurred in summer with the highest frequency (75.6%), at home (81%) and at night (54%). No autonomic storm, pulmonary edema due to heart failure or non-cardiac reasons, and heart failure developed in any of the patients. None of the cases needed mechanical ventilator and did not die. No severe allergic reactions were observed in severe cases to whom antivenom was administered. Conclusion: Although mortality rates due to this reason are low, scorpion bites are still an important problem in our country and region. Therefore people should be educated how to take protective measures, and they should apply to health institutions as soon as possible. Keywords: Scorpion; Child; Intensive Care
