Giant Axonal Neuropathy with Predominant Central Nervous System Manifestations

SUMMARY The authors describe a 25-year-old woman with giant axonal neuropathy (GAN) and severe CNS involvement. She had been admitted to hospital with generalized seizures, and had had gait disturbances followed by progressive mental deterioration since childhood. Neurological examination revealed mental retardation, scanning speech, cerebellar dysfunction, pyramidal signs, mainly in the lower extremities, and peripheral sensory neuropathy. Sensory nerve conduction velocity was decreased; brain CT and MRI showed diffuse demyelination. Sural nerve biopsy revealed characteristic signs of GAN. The patient's older sister had died at the age of 23, after having had similar neurological disturbances since childhood. This case illustrates an unfamiliar presentation of GAN, characterized by mild sensory neuropathy and severe CNS involvement, including seizures. RESUME Neuropathie a axones geeants avec manifestations predominantes dans le systeme nerveux central Les auteurs decrivent le cas d'une jeune femme de 25 ans presentant une neuropathie a axones geants (GAN) et une atteinte severe de SNC. Elle avail ete hospitalisee pour crises comitiales generalisees et presentait une alteration de la demarche survenue apres une deterioration mentale progressive depuis l'enfance. L'examen neurologique precisait le retard mental, revelait une parole scandee, des troubles cerebelleux, des signes pyramidaux predominant aux membres inferieurs et une neuropathie sensorielle peripherique. La vitesse de conduction dans les nerfs sensoriels etait abaissee, le scanner cerebral et l'IRM demontraient une myelinisation diffuse. Une biopsie du S.P.E. revelait les signes caracteristiques de GAN. Une soeur plus âgee de la patiente etait morte a l'âge de 23 ans, apres des troubles neurologiques identiques durant l'enfance. Ce cas illustre une forme peu frequente de GAN, caracterisee par une neuropathie sensorielle moderee et une atteinte severe du SNC. Sie war wegen schwerer generalisierter Krampfe ins Krankenhaus gekommen und hatte seit ihrer Kindheit Gangstorungen und eine progressive geistige Behinderung. Die neurologische Untersuchung ergab geistige Retardierung, skandierende Sprache, Kleinhirndysfunktion, Pyramidenzeichen, hauptsachlich in den unteren Extremitaten, und periphere sensorische Neuropathie. Die Leitgeschwindigkeit der sensiblen Nerven war vermindert, Hirn CT und MRI zeigten diffuse Entmarkung. Die Biopsie des N. suralis ergab charakteristische Befunde der GAN. Die altere Schwester der Patientin war im Alter von 23 Jahren gestorben, nachdem sie eine ahnliche neurologische Erkrankung von Kindheit an hatte. Dies ist ein ungewohnlicher Fall einer GAN, der durch eine leichte sensorischen Neuropathie und eine schwere ZNS Beteiligung charakterisiert ist. RESUMEN Neuropatia axonal gigante con predominio de manifestaciones en el sistema nervioso central Los autores describen el caso de una mujer de 25 anos de edad con neuropatia axonal gigante (NAG) y grave afectacion del SNC. Habia ingresado en el Hospital con convulsiones generalizadas y habia tenido alteraciones de la marcha seguidas de deterioracion mental progresiva desde la infancia. El examen neurologico revelo un retraso mental, palabra escandida, disfuncion cerebelosa, signos piramidales, sobre todo en las extremidades inferiores y una neuropatia sensorial periferica. La conduccion nerviosa sensitiva estaba disminuida y la TAC cerebral y la IRM mostraban una desmielinizacion difusa. La biopsia del nervio sural revelo signos caracteristicos de una NAG. Una hermana mayor de la paciente fallecio a los 23 ans habiendo presentado unas alteraciones neurologicas semejantes desde el nacimiento. Este caso ilustra una forma inusual de presentacion de la NAG, que se caracteriza por una neuropatia sensitiva moderada y una grave alteracion del SNC.
