Purpose: The purpose of this study was to compare outcomes of arthroscopy versus osteochondral autologous transplantation (OAT) in patients with osteochondral lesions of the talus. Methods: We reviewed, retrospectively, 65 patients who had undergone surgical treatment for osteochondral lesion of the talus between 1988 and 2004. The mean age of the patients was 29, l years. After analysis of the lesion the patients were classificated and underwent arthroscopy or mosaicplasty. Outcomes were measu red with the use of the American orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale. Results: Sixteen patients had arthroscopy and 44 patients had OAT. Mean time to follow-up was 36.5 months. (Range 12 to 120 months) AHS score showed 88,9 points in the arth roscopy group, and 90 points in mosaicplasty. Conclusion: Our study demonstrate good results during the long follow-up.
