Purpose: The cognitive-behavioral units (UCC) are dedicated to Alzheimer’s disease patients with a worsening of behavior disorders. The aim is to describe pharmaceutical interventions performed by the clinical pharmacist during prescriptions analysis. Methodology: All patients hospitalized in the UCC (Hospices de Lyon, France) during 2011 were included. The clinical pharmacist performed prescription analysis and participated in multidisciplinary review. Results: Ninety-seven patients were included in the study. Among the 560 analyzed prescriptions, 190 pharmaceutical interventions were performed by clinical pharmacist and 148 were accepted (77.9%). Most frequently detected drug-related problems were nonconformity to guidelines or contraindication (39.9 %), dose adjustment (19.5%), and inappropriate administration route (17.6%). A substitution was proposed for 32% of pharmaceutical interventions. Conclusion: This study shows the impact of clinical pharmacist integrated to the multidisciplinary staff in order to improve drug prescribing and decrease iatrogenic risk in elderly.
