best of our knowledge, the attack rate for CDI cases has not been studied previously. The attack rate is probably related to cross-transmission of C. difficile strains, antimicrobial consumption, and predisposing conditions of patients admitted. Our study has several limitations. The toxin assays, although widely used, have limited sensitivity. These were the only techniques used in other studies. However, even when estimating the number of cases that might have been diagnosed by using the most sensitive tools available, the incidence would have been low. In addition, the epidemiology would be better characterized by using molecular techniques. As far as we are aware, there are no studies comparing the efficiency of different surveillance strategies for CDI. Active surveillance of patients with DPCCD and educational activities may be helpful, and cost analysis of such interventions in hospitals with low incidence should be performed.
