To the Editor: With great interest we read the article by Cardinale et al,1 who randomized 114 cancer patients with elevated troponin I to either enalapril 20 mg/d or no treatment 1 month after high-dose chemotherapy. The investigators found that patients treated with angiotensin-converting enzyme inhibitors (ACEI) had an impressive reduction in cardiotoxicity (defined as a decline in left ventricular ejection fraction >10% below 50%) compared with those who did not receive treatment.
