The behavior of glycemic parameters during the first quarter of pregnancy in women with type 1 diabetes mellitus Comportamiento de los parámetros glucémicos durante el primer trimestre del embarazo en mujeres con diabetes mellitus tipo 1

2009
Introduction: It is known that there is a higher frequency of hypogly- cemia during the fi rst trimester of pregnancy in women with type 1 diabetes mellitus: Patients and methods: We undertook a retro- spective longitudinal study of 64 pregnant women with type 1 diabe- tes mellitus. Four periods were defi ned: preconception (28 days prior to conception), fi rst, second and third month of pregnancy. Different glycemic parameters between those 4 periods were compared. Re- sults: Mean blood glucose level was similar in the preconception period and in the fi rst month. Afterwards, it progressively decreased during the second and third months. HbA 1c showed a similar tenden- cy as the mean blood glucose. Both non-serious hypoglycemia as well as nocturnal hypoglycemia increased throughout the fi rst trimes- ter. There were no signifi cant changes either in the number of daily blood glucose measurements or in the average daily dose of insulin. Conclusion: Together with an improved glycemic control, a tendency towards hypoglycemia -daytime as well as nocturnal- during the fi rst quarter of pregnancy in women with type 1 diabetes is corroborated and does not seem to be attributable to the dose of insulin.
