Analysis on related risk factors of concurrent contrast induced nephropathy after percutaneous coronary intervention

2018
Objective To analyze the related risk factors of concurrent contrast induced nephropathy after percutaneous coronary intervention, and to make targeted nursing countermeasures. Methods Clinical data of 175 patients with PCI operation was analyzed retrospectively, and the incidence of contrast induced nephropathy(CIN) after operation was figured. The single factor analysis and multivariate Logistic regression analysis were carried out on the factors affecting CIN after operation, and targeted nursing countermeasures were formulated according to the analysis results. Results In 175 cases of patients, there were a total of 24 cases with CIN after operation, with the incidence of 13.71%.Univariate factor analysis showed that there were significantly differences in such factors' constituent ratios as age, diabetes mellitus, renal insufficiency, acute myocardial infarction, dose of contrast agent, CIN incidence of patients with cardiac dysfunction(P<0.05). Multivariate analysis showed the factors of influencing the occurrence of PCI after CIN, according to the degree of risk, were renal insufficiency(OR=6.49), dose of contrast agent more than or equal to 250 ml(OR=5.48), acute myocardial infarction(OR=4.53), cardiac insufficiency(OR=3.96), diabetes mellitus(OR=3.13), and 60 years of age or older(OR=2.60) in turn. Conclusions Age, contrast dose, diabetes mellitus, acute myocardial infarction, etc., are the risk factors for the occurrence of CIN after PCI, and the specific situation of patients should be comprehensively analyzed and targeted nursing countermeasures should be formulated.
