Diagnostische Bedeutung des Tumormarkers CA 19-9 beim Pankreaskarzinom

Der diagnostische Stellenwert des tumorassoziierten Antigens CA 19-9 wurde bei 309 Patienten uberpruft, und zwar bei 156 Patienten mit maligner Erkrankung (39 mit Pankreaskarzinom, 117 mit extrapankreatischen Malignomen) und bei 153 Patienten mit benigner Erkrankung (54 mit chronischer Pankreatitis, 25 mit akuter Pankreatitis, 74 mit allgemeinchirurgischen Erkrankungen). Dabei ergaben sich Werte oberhalb des Normbereiches von 6-37 U/ml bei 95 % der Patienten mit Pankreaskarzinom (Medianwert 528 U/ml), bei 31 % der Patienten mit Magenkarzinom und bei 16 % der Kranken mit kolorektalen Karzinomen (Medianwert 8 U/ml). Bei der praoperativen Bestimmung der CA-19-9-Konzentration liesen sich eine Sensitivitat fur das Pankreaskarzinom von 95 %, eine Spezifitat von 87 % und eine Gesamt-Treffsicherheit von 88 % errechnen. Die praoperativ erhohte CA-19-9-Konzentration bei Pankreaskarzinom-Patienten fallt nach kurativer Resektion des Karzinoms bis zum Normbereich ab. Bei keiner CA-19-9-Bestimmung nach palliativem Eingriff oder bei inoperablem Karzinom konnte jedoch eine Serumkonzentration unter 37 U/ml gemessen werden. The diagnostic value of the tumour-associated antigen CA 19-9 was tested in 309 patients, 156 with malignancies (39 carcinoma of the pancreas, 117 extrapancreatic malignant tumour) and 153 patients with nonmalignant disease (54 chronic pancreatitis, 25 acute pancreatitis, 74 general surgical disease). Levels above the normal range of 6-37 U/ml were obtained in 95 % of patients with carcinoma of the pancreas (mean value 528 U/ml), in 31 % of patients with gastric carcinoma and 16 % with colorectal carcinoma (mean 8 U/ml). Results of the pre-operative determination of CA 19-9 indicated for carcinoma of the pancreas a sensitivity of 95 %, a specificity of 87 % and an overall accuracy of 88 %. Pre-operatively elevated CA 19-9 concentrations in patients with carcinoma of the pancreas fell to normal after curative resection of the neoplasm. After palliative operation or in inoperable carcinoma CA 19-9 serum concentrations were never less than 37 U/ml.
