A Study in Experiment and Clinic of Relativity Factors and Acting Mechanism on Portal Hypertensive Gastropathy(PHG)with Cirrhosis

2001
Objective This paper explores the relativity factors and acting mechanism on PHG to be applied clinical medical treatment Methods Sixty cases patients were biopsied gastric mucosal tissue by electric-gastroscopy,to test H Pylori by using various means;to study gastric mucosal structure changes by using transmisson electron microscope;to determine peripheral blood concentratin of Gastrin、Mtl、ET、IL-2 and TNE by RIA;to detect hepatic function status (HFS) by child-pugh A、B and C;the portal venous flow (PVF) status;to test esophageal varicosity forms Results PHG occurrence and HFS,PVF,H Pylori infection are interrelated;PHG occurrence and esophaged varicosity form, with and without upper digestive Tract Hemorrhage (UDTH)history has no related; The concentration changes of Gastrin,Mtl,ET,IL-2 and TNF in peripheral blood are interrelated with the development and occurrence of PHG;The union of ante-infective H Pylori, protecting from liver drugs and reducing PVP drugs can obviously decrease or relax gastrointestinal symptoms of PHG patients Conclusion Three major factors in the pathogenesis of PHG are H Pylori infection,PVF and HFS;H Pylori infection with the portal hypertensive was by the mechanism of influencing gastrointestinal physiology and mucosal immunopathological injuring that contributed to the development and occurrence of PHG
