Angiotensina II y PCR en pacientes con síndrome metabólico e hipertensión

El Dr. Reaven sugirio que la resistencia insulinica desempenaba un papel central en el desarrollo de diabetes tipo 2,hipertension y enfermedad arterial cardiovascular; por tal motivo el objetivo de este estudio fue determinar niveles sericos de Angiotensina II y PCR, en pacientes con Sindrome Metabolico e hipertension. La poblacion corresponde a 30 pacientes 15 controles y 15 con Sindrome Metabolico e hipertension, que consultan al Departamento de Endocrinologia y Enfermedades Metabolicas del Hospital Militar “Dr. Carlos Arvelo” entre Abril-Mayo del 2009. A todos los pacientes se les determino: glicemia, creatinina, acido urico, colesterol total, colesterol HDL, colesterol LDL, trigliceridos y Proteina C reactiva por metodo enzimatico colorimetrico, kit comercial Roche Diagnostic C.A e Insulina por metodos de quimioluminiscencia de Roche Diagnostic C.A, Angiotensina II por kit de SPI Bio,para la estimacion de la sensibilidad insulinica, se determino por modelo matematico por el Homeostasis Model Assessment (HOMA). Se aplico la prueba “t” de Student para muestras evaluadas, utilizando el programa informatico SPSS® version 2009. Se evidencio un incremento significativamente estadistico (p<0,05) de los valores de Angiotensina II y PCR en los pacientes con Sindrome Metabolico, con hipertension, en comparacion con los sujetos controles. En los sujetos con sindrome metabolico se demostro un incremento de la Angiotensina II a medida que se aumenta la presion arterial sistolica, signo este de disfuncion endotelial y por ende contribuye a elevar el riesgo de enfermedad cardiovascular. Dr. Reaven has suggested that insulin resistance (IR) is playing a central role in the development of diabetes type 2, hypertension and cardiovascular arterial disease. The goal of the study was to determine the serum levels of Angiotensin II and PCR, in patients whom suffering of Metabolic Syndrome and hypertension. The population is conformed up to 30 patients. 15 controls and 15 whom are suffering metabolic syndrome and hypertension, which were attended in the consult at the Department of Endocrinology and metabolic diseases of the military Hospital “Dr. Carlos Arvelo” between April-May 2009., It was determined, in each one of the patients: blood glucose, creatinine, uric, acid total cholesterol, HDL cholesterol, cholesterol LDL, triglycerides. It was also determined on them the protein C-reactive by using the enzymatic-colorimetric method with the commercial kit of Roche Diag22 nostic C.A, and insulin using of chemiluminescence from Roche Diagnostic C.A, Angiotensin II with the kit SPI from Bio for the estimation of insulin sensitivity, determined by mathematical by Homeostasis Model Assessment (HOMA) model. The data was analyzed by applying the Student’s t- test, using the computer program SPSS® version 2009. There was observed a significant statistical increase (p < 0.05) value of angiotensin II and CRP in patients who are suffering metabolic syndrome and hypertension as compared with patients used as control. Patients with metabolic syndrome have showed an increase of angiotensin II, as a function of the systolic blood pressure increment. This is a sign of endothelial dysfunction, and thus contributes to increase the risk of cardiovascular disease.
